Refining prognosis after trans-arterial chemo-embolization for hepatocellular carcinoma

2015

Background & aims To develop an individual prognostic calculator for patients with unresectable hepatocellular carcinoma (HCC) undergoing trans-arterial chemo-embolization (TACE). Methods Data from two prospective databases, regarding 361 patients who received TACE as first-line therapy (2000-2012), were reviewed in order to refine available prognostic tools and to develop a continuous individual web-based prognostic calculator. Patients with neoplastic portal vein invasion were excluded from the analysis. The model was built following a bootstrap resampling procedure aimed at identifying prognostic predictors and by carrying out a 10-fold cross-validation for accuracy assessment by means o…

Radiomics: A New Biomedical Workflow to Create a Predictive Model

2020

‘Radiomics’ is utilized to improve the prediction of patient overall survival and/or outcome. Target segmentation, feature extraction, feature selection, and classification model are the fundamental blocks of a radiomics workflow. Nevertheless, these blocks can be affected by several issues, i.e. high inter- and intra-observer variability. To overcome these issues obtaining reproducible results, we propose a novel radiomics workflow to identify a relevant prognostic model concerning a real clinical problem. In the specific, we propose an operator-independent segmentation system with the consequent automatic extraction of radiomics features, and a novel feature selection approach to create a…

Use of Machine Learning and Artificial Intelligence to Drive Personalized Medicine Approaches for Spine Care

2020

Personalized medicine is a new paradigm of healthcare in which interventions are based on individual patient characteristics rather than on “one-size-fits-all” guidelines. As epidemiological datasets continue to burgeon in size and complexity, powerful methods such as statistical machine learning and artificial intelligence (AI) become necessary to interpret and develop prognostic models from underlying data. Through such analysis, machine learning can be used to facilitate personalized medicine via its precise predictions. Additionally, other AI tools, such as natural language processing and computer vision, can play an instrumental part in personalizing the care provided to patients with …

A complication risk score to evaluate clinical severity of thalassaemia syndromes

2020

The thalassaemia syndromes (TS) show different phenotype severity. Developing a reliable, practical and global tool to determine disease severity and tailor treatment would be of great value. Overall, 7910 patients were analysed with the aim of constructing a complication risk score (CoRS) to evaluate the probability of developing one or more complications. Nine independent variables were included in the investigation as predictors. Logistic regression models were used for Group A [transfusion-dependent thalassaemia (TDT)], Group B [transfused non-TDT (NTDT)] and Group C (non-transfused NTDT). Statistically significant predictors included age (years), haemoglobin levels, hepatic transaminas…

A Novel Approach to Identify Candidate Prognostic Factors for Hepatitis C Treatment Response Integrating Clinical and Viral Genetic Data

2015

The combined therapy of pegylated interferon (IFN) plus ribavirin (RBV) has been for a long time the standard treatment for patients infected with hepatitis C virus (HCV). In the case of genotype 1, only 38%–48% of patients have a positive response to the combined treatment. In previous studies, viral genetic information has been occasionally included as a predictor. Here, we consider viral genetic variation in addition to 11 clinical and 19 viral populations and evolutionary parameters to identify candidate baseline prognostic factors that could be involved in the treatment outcome. We obtained potential prognostic models for HCV subtypes la and lb in combination as well as separately. We …

Prognosis of resected intrahepatic cholangiocarcinoma

2017

Introduction. Complete resection stands as the only curative option for intrahepatic cholangiocarcinoma (IHCC). Still, prognosis remains poor after resection due to a recurrence rate over 60% leading to actual 5-year survival rates below 20%. Reliable prognostic estimation and better understanding of tumor biology would be of interest for improving IHCC prognosis.Methods. Using clinical and biological data from two large cohort of resected IHCC (MSKCC, n=189 and AFC, n=522), three objectives have been explored. First, assessing the performances of different published prognostic models. Second, defining the reliability of preoperative prognostic estimation using imaging, tumoral genomic prof…

Economic value of a local museum

2004

Abstract The aim of this paper is to determine the economic value of a local cultural history museum, namely, the Museum of Central Finland in Jyvaskyla. This study also seeks to clarify what factors affect the willingness-to-pay for the Museum. Data were gathered from a sample of Jyvaskyla residents aged 18 and over via a postal questionnaire in November and December 1997. The results indicate that in reality, Jyvaskyla residents contribute less in taxes to the Museum than they report that they are willing to pay. This indicates that at least the present amount of tax revenue can justifiably be directed to the Museum. Maintenance of the Museum can thus be legitimised on the basis of the pu…

Standardisierte pathoanatomische Befunddokumentation auf den ADT-Tumorbgen fr Malignome des Mundes, der Kiefer und des Gesichts (Version III)

1995

The documentation form for pathohistologic findings (version III) for tumors of the maxillofacial region is presented. It is part of the site-specific documentation of the German Association of Tumor Centers (ADT). Its handling is explained by instructions which are based on the ICD-O classification and the TNM system. The prognostic relevance of the standardized documentation has already been proven by various histomorphologic investigations concerning oral and oropharyngeal cancer. By means of a multicentric observational study the German Austrian Swiss co-operative group DOSAK will develop a new prognostic model for oral and oropharyngeal cancer.

Comparison of prognostic models in patients with advanced Hodgkin disease

2001

Several prognostic systems have been elaborated for patients with Hodgkin disease (HD) over the last 12 years, but early identification of a reasonably large group of both low and high risk, advanced stage patients remains unsatisfactory.Seven well known models were applied to 516 patients with advanced HD, with 315 patients used for the study sample and 201 patients used for the test sample. Individual performances as well as joint performances were analyzed univariately and multivariately in relation to overall survival, recurrence free survival, and time to treatment failure by means of a proportional hazards model.None of the models identified a group containing10% of patients from the …
